The most efficient way to request an Endorsement or Cancellation is via email or facsimile to your Production or Account Management Team Member (see below). The following information is required for Endorsement Requests, (signed LPR’s are required on Cancellation Requests):

  • Insured Name, including DBA (if applicable)
  • Policy Effective Dates
  • Policy Number
  • Complete description of the requested Endorsement with any specific and additional information needed to process that request.

Endorsement Requests are processed in order they are received. Please do not resend a request – contact our office for a status. Endorsements and invoices (if applicable) will be e-mailed back to the Agent making the request and not to the insured.
